A US school bus crash that injured 19 students has reignited debate around school bus safety tech, with investigators calling for alcohol detection systems and stronger occupant protection.
The National Transportation Safety Board (NTSB) found that a March 4, 2024, rollover in Millstone, West Virginia, was caused by a driver operating the vehicle while significantly impaired by alcohol. The crash left three students seriously injured and exposed gaps in both driver monitoring and passenger safety design.
The bus, carrying 19 students, left the roadway while navigating a curve, struck a culvert and rolled onto its side.
“The probable cause of the Millstone, West Virginia, crash was the school bus driver’s alcohol impairment resulting in his loss of vehicle control,” the NTSB said.

The driver’s blood alcohol concentration was measured at 0.161 grams per decilitre shortly after the crash, more than four times the legal limit for commercial drivers.
The findings have prompted renewed calls for school bus safety tech that is capable of actively preventing impaired driving before a vehicle even moves.
“We found that implementing alcohol detection systems on school buses can prevent alcohol-impaired driving by school bus drivers,” the NTSB said.
Such systems already exist in parts of Europe, where ignition interlock technology prevents a vehicle from starting if alcohol is detected. However, no US state currently mandates this technology for school buses.
The NTSB has now recommended that the National Highway Traffic Safety Administration require all new school buses to be equipped with integrated alcohol detection systems that can block operation when impairment is detected.
The investigation also highlighted a second, equally critical gap in school bus safety tech: the absence of passenger seat belts.
Despite the bus sustaining limited structural intrusion, unbelted students were thrown around the cabin during the rollover sequence, increasing injury risk.
“We also found that unbelted bus passengers were injured from impacting other occupants and the school bus interior,” the NTSB said.
The bus was not equipped with lap or shoulder belts for passengers, which remains legal under US regulations for large school buses.
In both the West Virginia crash and a separate Texas rollover examined by investigators, unrestrained passengers were displaced from their seating compartments, struck hard surfaces, and in some cases risked ejection.
“Lap/shoulder belts, had they been installed and properly worn, would have provided the best protection for the students,” the NTSB said.
The agency has reiterated its long-standing recommendation that states mandate lap and shoulder belts on new school buses, along with enforceable policies to ensure they are actually used.
While school buses remain one of the safest modes of transport overall, the scale of daily operations adds weight to these findings. Across the United States, around 25 million students rely on school bus services each day.
Investigators argue that targeted upgrades to school bus safety technology, particularly in driver impairment detection and occupant restraint systems, could significantly reduce the risk of serious injury in the relatively rare but high-impact incidents that do occur.
